What Causes Leader Accountability Issues

Accountability
When leaders and team members don't see how they are accountable to themselves, their team, and the organization as a whole when they are in isolation, personal achievements will trump organizational achievements.
Performance Measures
When leaders and team members understand performance measures to only be based on personal success and not on organizational success, the shared goals will not be the focus.
Feedback, not Feed-Forward
When leaders and all employees only receive feedback on what they did wrong and not "feed-forward" on what they can do better, they will be less likely to share and collaborate with others, losing sight of organizational goals.
